Athletic Resources & Forms

The Athletic Participation Form must be on file in the Athletic Office before students are allowed to participate in tryouts or practice for any sport.  This form is valid for the entire school year.
Athletic Participation Form

The Westminster/GHSA Physical Form is an annual requirement for all students grades 6th through 12th, who participate in athletics. It is to be completed and signed by a licensed medical physician or doctor of osteopathic medicine, and submitted to the Athletic Office.

This form must be submitted to the Athletic Office for 6th through 12th grade students ONLY prior to participating in athletics.

Westminster/GHSA Physical Form

Students must have BOTH the Athletic Participation form and the Westminster/ GHSA Physical form on file before participation in athletics is permitted.

The Student-Parent Athletic Handbook is required reading for every athlete and their parents.  It contains important information including Department of Athletic policies, GHSA rules, eligibility and hazing.
